I think this should do it:

album.displayFormat={$album^$date^ (^)}, title.displayFormat={$title^ $composer^ (^)}, artist.displayFormat={$conductor^$albumartist^ (^)$discnumber^ (Disk ^)}

You also need to add Conductor to either itemTags or indexTags.
