Assuming you have a new-ish version of the app, select 'Send Feedback' from the navigation drawer. Then tap the 'Include system info' checkbox. Under the 'Media Servers' section you'll see all the UPnP servers that you've ever selected in the app. Assuming you've selected the Online instance you referred to I assume you'll see two entries in there for the same server.

The main reason that offline instances are not removed is because Hi-Fi Cast remembers the server that you previously selected - so it has to be listed in the 'Media Servers' screen even if it is unreachable at the time (because that screen shows the currently selected server). Also, I think it makes for a more consistent user experience to see all of the servers listed rather than the list changing depending on the discover-ability of the servers.

It shouldn't however show the same server twice so I don't know what's going on there.
